Overview: The Door is an unparalleled model for youth development, offering a comprehensive range of integrated services within a single site for nearly 10,000 New York City youth each year. Our mission is to empower young adults to reach their full potential by providing comprehensive youth development services in a diverse and caring environment. At The Door, youth can access health care and education, mental health counseling and crisis assistance, legal assistance, college preparation services, career development, housing supports, arts, sports and recreational activities, and nutritious meals – all for free and under one roof. By providing participants with our suite of integrated services, we seek to provide any motivated young person with the tools, resources, and opportunities needed to successfully transition to adulthood.

The PrEP Navigator provides intensive care coordination, support and services to HIV negative individuals who require assistance in accessing and remaining in PrEP/PEP care. This individual will coordinate with the Adolescent Health Center health education, medical and entitlements teams to initiate access to PrEP/PEP care and provide assistance to existing PrEP/PEP patients. This position will assist PrEP/PEP clients with services such as, but not limited to, PrEP/PEP screening, health insurance enrollment, appointment scheduling, appointment reminders, education on the benefits of PrEP and ongoing maintenance. This position will work to empower clients to navigate medical and social systems on their own after aging out of The Adolescent Health Center.

Responsibilities:

  • Work with Health Center staff and other agency staff to identify potential PrEP/PEP clients
  • Provide support to HIV negative individuals who are interested in starting PrEP/PEP and those already utilizing PrEP/PEP care
    • Complete screenings to identify readiness for PrEP/PEP and barriers to care. Work with client to identify solutions.
    • Assist with benefits programs and insurance enrollments
    • Provide prevention and PrEP adherence education and counseling
    • Effectively collaborate with all individuals involved in PrEP/PEP care and program delivery for the purposes of supporting high quality PrEP/PEP programming.
    • Make referrals for internal and external services needed to support PrEP care and document follow up
    • Provide ongoing follow up to ensure PrEP/PEP adherence
  • Maintain appropriate patient files and timely documentation of services utilizing electronic medical records and logs
  • Individual and group education/counseling with adolescents around health topics
  • Provides HIV education, testing and pre/post-test counseling
  • Provide outreach to relevant communities with a focus on young men who have sex with men
  • Establish working relationships with appropriate community resources in The Door, including those providing mental health and social services
  • Represent The Door and its philosophy to outside agencies, schools, other health facilities, CBOs, health fairs and community meetings
  • Attend required trainings and meetings
  • Other tasks assigned by supervisor

Qualifications and Requirements:

  • Bachelor’s Degree in related field preferred, High School Diploma or HSE required
  • Minimum two years of experience with providing PrEP/PEP/HIV benefits navigation
  • Knowledgeable of and ability to apply positive youth development principles to working with young people
  • Demonstrate an understanding and practice of the principles of positive youth development
  • Understanding of and sensitivity to HIV/AIDS and PrEP/PEP
  • Demonstrated experience with electronic health records (preferably eClinicalWorks) and proficiency with Excel
  • Ability to closely collaborate with colleagues
  • Ability to work well under pressure and meet deadlines
  • Excellent verbal and written communications skills
  • Demonstrated experience with urban LGBTQ youth
  • Must be a professional and responsible person who is able to develop and maintain interagency liaisons as a means of meeting client needs
  • Proficiency in Spanish, French, or Mandarin

Hours: 35 hours per week with some evening hours
Salary: Mid – High 30’s, plus generous benefits package
